Frequently Asked Questions

Answers to the common questions in our office

At FROGS, we have many excellent relationships with local family physicians, orthopedic surgeons, physical medicine physicians, neurologists and many additional providers.

WHAT TYPE OF INSURANCE DO YOU ACCEPT?

We accept all major PPO insurance companies. Please call our office to identify your insurance coverage.  We will be happy to pre-certify your insurance coverage.  Our staff will be able to give you what your insurance coverage and your out of pocket expenses will be.  We will work with your on an individual basis to make sure you will have a quality experience at FROGS Physical Therapy. We utilize a billing company but we track internally what we bill insurance, what they will allow, what your deductible balance is and what insurance pays and what your per visit charge will be.

WHAT IS PHYSICAL THERAPY?

Physical therapy is a health care profession that helps individuals whose movement and function are adversely affecting by aging, injury, disease, or environmental factors.  Together with the help of physical therapists, these patients work toward the goals of relieving pain, as well as developing, maintaining, and restoring maximum movement and functional ability.

The cornerstones of physical therapy treatment are  physical assessment, neuromuscular balance, core stability, therapeutic exercise, manual therapy, and functional training.  Physical therapy, which has many specialties such as sports medicine, industrial, orthopedic, and pediatrics, is available in many settings: outpatient clinics, offices, inpatient rehabilitation facilities, extended care facilities, private homes, schools, hospitals, industrial workplaces, and fitness centers.

WHAT DO PHYSICAL THERAPISTS DO?

Physical therapists are experts in restoring movement and function.  After examining patients, therapists create comprehensive programs focused on total body care, injury prevention, as well as safe and timely returns to work, life, or sports.  Depending on the needs of a particular patient, physical therapists may mobilize a joint, provide soft tissue work to a muscle, or perform a corrective exercise to restore normal movement patterns and function.

In addition to this hands-on care, physical therapists educate patients on preventing injury and performing home exercises.

HOW DO I RECEIVE PHYSICAL THERAPY

There are several options:  1) Direct referral from Physician;  2) Direct Access: Directly go to a physical therapist for an evaluation and care  3) Workman’s Comp: Injury at work  4) Motor Vehicle Accident: referred by Doctor or Lawyer

An individual has the right to choose where they want to receive physical therapy.  A doctor’s office may recommend a physical therapy clinic or provide you with a list of recommended providers.  Most people choose a PT clinic based on where they live or work.

An individual should look at what PT clinic is going to provide me with the best care and have the expertise to treat my specific condition.

One way is to check reviews on line such as Google, Google plus, Yahoo etc

An individual or group insurance plan has “in network providers” and “out of network providers”.  It is important to know your benefits because an “out of network” provider may provide better care with not much more cost.

Be sure to let your health care provider or insurance company know that FROGS Physical Therapy is your first choice when it comes to rehabilitation services. We participate in many insurance plans; however, if FROGS Physical Therapy is not in your network, our staff can check on your in network and out of network benefits and provide you with the information necessary to make an informed decision.  We will work with your out of network benefits and make sure you will not have unnecessary expenses.  We appreciate you interest in FROGS Physical Therapy, and we’d love to work with you on your return to an active lifestyle.

Physical therapy is valuable in the treatment of many diagnoses, so it’s always a good idea to ask your health care provider to prescribe physical therapy before other methods such as prescription medication. If you are unsure whether or not physical therapy can help you, contact FROGS Physical Therapy for an initial consultation.

FROGS Physical Therapy has morning and afternoon appointments available to meet the demands of your busy schedule.  Appointments at our North Scottsdale location can be made calling Monica at 480-585-7300 or emailing us at clarya@frogspt.com.

WILL THE PHYSICAL THERAPIST COMMUNICATE WITH MY HEALTH PROVIDER?

FROGS Physical Therapy has morning and afternoon appointments available to meet the demands of your busy schedule.  Appointments at our North Scottsdale location can be made calling Monica at 480-585-7300 or emailing us at clarya@frogspt.com.

Please have your prescription and insurance card available when you call.  This will allow us to verify your insurance benefits and provide you with an estimate of coverage.

FROGS Physical Therapy recognizes that open, honest communication among the members of your health care team (you, your therapist, your health care provider, etc.) is essential to making your rehabilitation experience a success.  As a result, the physical therapist will send your health care provider a complete written report that includes the findings of the initial evaluation, the assessment of the injury, and a detailed description of the treatment plan.

The physical therapist will continue to update your health care provider with written progress reports each time you are re-evaluated and before each follow-up appointment (so it’s important to let your therapist know if you have an upcoming appointment with your health care provider).

Additionally, the therapist will not hesitate to contact your provider by phone if he/she has a specific concern or question.  At the end of your treatment, the physical therapist will send a final report to your health care provider with information regarding your status at the time of discharge from physical therapy.

WHAT TYPE OF FOLLOW UP IS OFFERED?

At FROGS Physical Therapy, our well-trained, professional staff emphasizes the importance of continuing your treatment with a home exercise program.  Your therapist will design an “at-home care” program for you and provide detailed instructions.  This will allow you to continue the progress you made while therapy and avoid further injury.

We also have a step down program for patients who want some additional supervision with their exercise program.  Graduate Fitness Program is a 2-3 day per week program for a month at a time and a person can renew up at the end of each month.  Call for further information.

We are partners in your recovery; so even after discharge, if you have questions, call us and we will be more than happy to help your with any questions or needs you may have.

DO I ALWAYS NEED A PRESCRIPTION FOR PHYSICAL THERAPY?

No. Arizona is a direct access State.  This means if there is no restriction in your policy, you can seek physical therapy care without a prescription.  In order to continue physical therapy, there has to be a documented plan of care that would warrant physical therapy.

An individual can also pay out of pocket for physical therapy services without involving your insurance company.  Please talk with Monica if you have any questions.
